Applying to the Habitat Affordable Homeownership Program

 
  Documents for Individuals/Families Seeking to Partner with Habitat: image

Please read the following before filling out application:

  • Please read and complete the application in its entirety. Print clearly: any blank or illegible areas will result in the application being returned to you.
  • Both applicant and co-applicant must read and sign the "Authorization and Release of Information" form and the "Background Authorization" form.
  • Required supporting documents:
    • Copies of two forms of personal identification for each applicant
    • Copies of four most recent consecutive paystubs for each employed house member
    • Copies of any earned SSI, Child Support, Alimony, Pension Fund, Worker's Compensation
    • If you are currently married but separated from your spouse, we must consider him/her a co-applicant and will need all of the above documents from him/her as well.
