Dear Applicant:
- Please complete the
homeownership application entirely,
any blank areas will result in the application being returned to
you. Do not write on the sides or back of this form. If you need additional
space or would like to include additional information, please use a separate
piece of paper.
- The applicant and co-applicant both need to read
and sign the Authorization to Provide Credit Information section at the end
of the application. Each additional member of the household 18 years and
older who will live in the Habitat house will need to print out, read
and sign the Authorization to Provide Credit Information form
- Enclose a money order or personal check for
$16
for each adult (18 years or older) to cover the Credit Report fee.

Enclose copies of your tax returns for the past three years for each adult.
- Enclose copies of the four most recent pay
stubs for each adults in your household.
- Enclose a copy of your most recent utility
bill.
- If you rent, enclose a copy of your rental
lease or a copy of a money order receipt or cancelled check showing rent
payment.

If you are divorced, please send a copy of the divorce decree as well.
- Return the completed application with your check
or money order for the Credit Report(s) payment to:
