Please read the following before filling out application:
- Please read and complete the application in its
entirety. Print clearly: any blank or illegible areas will result in the
application being returned to you.
- Both applicant and co-applicant must read and sign
the "Authorization and Release of Information" form and the
"Background Authorization" form.
- Required supporting documents:
- Copies of two forms of personal identification
for each applicant
- Copies of four most recent consecutive paystubs
for each employed house member
- Copies of any earned SSI, Child Support,
Alimony, Pension Fund, Worker's Compensation
- If you are currently married but separated from
your spouse, we must consider him/her a co-applicant and will need all of
the above documents from him/her as well.
